Why and when your consent is necessary
When you register as a client of Tide Mill Therapy & Consulting , you provide consent for us to access and use your personal information so we can provide you with the best possible healthcare. If we need to use your information for anything else, we will seek additional consent from you to do this.

Why do we collect, use, hold and share your personal information?
We will need to collect your personal information to provide healthcare services to you. The main purpose for collecting, using, holding and sharing your personal information is to manage your health. We also use it for directly related business activities, such as financial claims and payments, practice audits and accreditation, and business processes (e.g. training).

The personal information we will collect includes your name, date of birth, address, contact details, health identifiers and, where applicable, your health fund details for identification and claiming purposes. We also need to gather information about your past and present health, in order to provide the best therapeutic care for you.

How is personal information collected?
Tide Mill Therapy & Consulting may collect your personal information in several different ways.

When you register for your appointment and in subsequent and during the course of providing therapeutic services.

When you visit the website, send an email or SMS, telephone us, make an online appointment or communicate with us using social media.

In some circumstances personal information may also be collected from other sources, such as your guardian or responsible person, other involved healthcare providers or your health fund.
Sharing your personal information
Tide Mill Therapy & Consulting respects your right to decide how your personal information is used or shared. All personal information gathered during the provision of this therapeutic service remains confidential and secure, and will only be shared other healthcare providers with your consent, except when

failure to disclose information would place the client or another person at risk, or

it is subpoenaed by a court, or

the client’s prior permission has been obtained to provide a written report to another professional or agency, or

permission has been obtained to discuss the material with a designated person.
We seek professional supervision as required by our professional association to maintain standards of practice. In doing so, we remove all identifying information about clients. In order to use client information for the purposes of training, education, research and publication, we will always seek your permission and ask you to sign a Consent Form. If you do not wish any of your information to be used in these areas, your wishes will be respected. This decision does not affect or compromise the therapeutic relationship.
Only people who need to access your information will be able to do so. Other than as otherwise described in this policy, this practice will not share personal information with any third party without your consent.
This practice will not use your personal information for marketing any of our goods or services directly to you without your express consent. If you do consent, you may opt out of direct marketing at any time by notifying our practice in writing.
This practice may use your personal information to improve the quality of the services it offers to clients through research and analysis of client data.
Storing and protecting your personal information
All personal information will be securely stored in password protected information systems and in hard copy format in a locked filing cabinet.
Accessing and correcting your personal information at Tide Mill Therapy & Consulting
You have the right to request access to, and correction of, your personal information.
We acknowledge clients may request access to their health records. We require you to put this request in writing via email and we will respond within 30 days. A fee of $250 will apply.
This practice will take reasonable steps to correct your personal information where the information is not accurate or up to date. From time to time, we will ask you to verify that your personal information held by this practice is correct and current. You may also request that we correct or update your information, and you should make such requests in writing to hello@tidemilltherapy.com.au
Privacy-related complaints
If you have any privacy-related complaints or concerns, please express these in writing to Tide Mill Therapy & Consulting at hello@tempotherapy.com.au or PO BOX 325, Christmas Island, 6798. We will work to resolve the matter within 30 days as per our resolution procedure. If you have any queries, please call us on 0461 265 112.

Privacy and my website
We will only collect personal information through our website if a person voluntarily provides it. For example, we may record a person’s name or email address if they send us an email or submit a web page form. We may collect non-identifying information in relation to the use of my website, including the type of browser used, the files requested, and the domain name and country from which the request arises. This information may be used to monitor and improve our website and services and for other purposes. Our website may use ‘cookies’ to improve the functionality of the website. The cookie will only identify the website visitor’s computer – it does not identify the visitor personally. Our website may contain links to other websites. This privacy policy has no application to any other website and Tide Mill Therapy & Consulting is not responsible for the practices or the content of any other website.
Policy review statement
This privacy policy will be reviewed yearly to ensure it is in accordance with any changes that may occur. Notifications of these changes will be made on the relevant social media channels and via email for currents clients.
